    Good to Know: Healthy Me Journey

    • Share:
      • We learned about HealthCorps’ Wellness Guidelines, which have been adapted from the 2015-2020 Dietary Guidelines for Americans to address HealthCorps’ three pillars of focus: nutrition, physical activity, and mental resilience.
      • To help you engage in these healthy behaviors, we are about to embark on the Healthy Me Journey (HMJ) together.
      • The HMJ is a program developed under a National Institutes of Health-National Institute of Diabetes and Digestive and Kidney Diseases (NIH-NIDDK) grant in partnership with the Albert Einstein College of Medicine to encourage people to make healthier choices by providing the knowledge and skills necessary to establish healthy habits.
      • Regardless of our current health status or how fit we are, we are all on our own journey to achieve and maintain good health.
      • Thus, pursuing a healthy lifestyle is personal, and to better understand where we currently stand, we will start our journey by taking an online health behavior questionnaire called the Healthy Me Survey.
      • Based on your answers, you will receive an individualized feedback report called Healthy Me Snapshot that indicates how your behaviors compare to the Wellness Guidelines.
      • I want you to remember that this report is called a “snapshot” because this only reflects your health behaviors at this point in time; it does not define who you are nor does it mean you can’t change.
      • This is just a starting point.
      • After you receive your Healthy Me Snapshot, we will set personal health goal using the SMART Goal structure. The SMART Goal structure will guide you to be intentional in your goal setting by making a goal specific, measurable, action-oriented, realistic and timely. We will work on our goals for the next several weeks, and we will check-in on our progress during class, support each other, and learn from one another.
      • At the end of the SMART Goal duration (4 to 12 weeks), we will reflect and think critically on our journeys.
      • We’ll think about how the journey changed us physically and mentally, discuss the barriers and challenges we faced, and of course, celebrate our success.
      • But the journey doesn’t end there! We hope that you will continue to set SMART goals not only to achieve health goals but also anything you want to achieve.

    Real World Relevance: Healthy Me Snapshot Survey

    • Share:
      • The Healthy Me Snapshot is a tool to learn more about yourself, and this is all about your health.
      • Once you complete the questions online, you will receive your Healthy Me Snapshot by email.
      • This snapshot is a personalized report about your habits related to what you eat and how active you are.
      • You will use this information in goal setting. Does anyone have any questions?
    • Instruction:
      • Hand each participant a slip of paper with their name and assigned ID number.
    • Share:
      • Since this Healthy Me Snapshot is completely private, we will not be using any names and will be using numbers instead.
      • Once you receive your slip of paper, log onto the website, and enter your unique number into the box that says ‘ID Number.’
      • Now, you are free to complete the Healthy Me Snapshot. Questions?
    • Instruction:
      • Have participants complete the Healthy Me Snapshot survey.
      • Please contact HealthCorps to receive an url link to this survey.
      • The survey should take approximately 10-15 minutes to complete.
      • While participants are taking the Healthy Me Snapshot survey, ensure participants have entered in the correct ID number and answer any questions.
      • Announce time remaining every 5 minutes.

    Hands On: Guideline Charades

    • Share:
      • Let’s play a game to remember and even memorize the Wellness Guidelines for the rest of your Healthy Me Journey.
    • Instruction:
      • Divide participants into groups and assign each a guideline.
      • Each group silently acts out the guideline for the rest of the groups to guess.
        • Decrease sugary beverage consumption
        • Increase frequency of breakfast consumption
        • Increase fruit and vegetable consumption
        • Decrease fast food consumption
        • Increase frequency of exercise
        • Decrease sedentary behavior
        • Increase conflict resolution skills
        • Decrease negative perception of challenges

    • Instruction:
      • Participants will have the option to write or draw in this activity.
      • Ask them to think about a time they set a goal for themselves.
      • This could be any goal (e.g. learning how to swim, getting better at playing guitar, learning how to cook, etc.).
      • o Have participants write/draw on their worksheet or answer the following question(s) out loud.
        • What were some challenges you faced when trying to meet this goal?
        • What steps did you take to meet your goal?
        • Did you end up achieving your goal?

      • The purpose of the activity we just did was to get you thinking about the idea of goal-setting.
      • You will soon receive the results from today’s survey, which will give you a snapshot of your overall health.
      • Based on this feedback, we are going to learn the best way to set personal goals - ones that that are fun, challenging but also reachable.
      • And don’t worry! All of the HealthCorps lessons we do during our Healthy Me Journey will help you achieve the goal(s) you set.
